The Center for Vision and Learning provides comprehensive vision care for children up to age 14. We also offer binocular vision evaluations, vision therapy, and vision rehabilitation for adult patients. To make your time with us as productive as possible, please print and complete the forms below and bring them with you to your appointment.

New patients - have you already had an eye exam by an optometrist or ophthalmologist within the past year? If so, please have your records faxed to the Center for Vision and Learning at 314-628-9191 at least 48 hours prior to your appointment with us.

Please feel free to provide copies of any reports pertaining to your child that may help our doctors understand how vision may be impacting their learning. These may include special education assessments, individual education plans (IEPs), reports from educational psychologists, occupational therapists, etc. Whenever possible, it is helpful for the doctors to have time to review these prior to your child's exam. Copies of the reports may be mailed to the address above, or faxed to 314-628-9191.
